ABSTRACT

The first reports of SARS-CoV-2 virus infection occurred in late 2019. Infection and the development of COVID-19 disease are directly related to the particular characteristics of the individual, such as gender, age, and comorbidities. Moreover, indi-vidualswho had some type of chronic disease expressed a higher rate of complications arising from the infection. This study aimed to verify the prevalence of comorbidities in individuals infected by COVID-19 in the city of Jaragua do Sul/SC from March 2020 to December 2021. The research presented a cross-sectional, descriptive, analytical design with a quantitative approach, carried out through secondary data using the Olostech in-formation system of the Health Department of the municipality. The results showed that 40,010 subjects were infected during the study period, of which 39,574 (98.9%) were recovered and 436 died (1.1%). It was observed a predominance of females in the recov-ered group (52.3%) and males in the deceased group (59.2%). The predominant age group in the group of recovered patients was 20-59 years (n=31,636;79.9%) and in the group of deaths it was 60 years or older (n=269;61.7%). In the year 2021 there were more cases of recovered (n=26,040;65.1%) and deaths (n=342;78.4%) when compared to the year 2020. The mean age in the recovered group was 37.5 +or- 15.8 years and in the death group was 63.2 +or- 15.7 years. The data showed the profile of infected individuals and the prev-alence of the main chronic diseases: hypertension, diabetes and obesity. We suggest ac-tions and strategies aimed at minimizing these comorbidities, aiming at a better quality of life for individuals in this city.

ABSTRACT

Primary healthcare (PHC) is a fundamental pillar in a health system, and its role has only been enhanced in the COVID-19 pandemic. The emergence of COVID-19 required health systems and, in particular, PHCs to develop a constant capacity to adapt and resist adversity. Therefore, it was considered important to identify innovative measures and thus the experience of a family health unit (USF) in Portugal – USF Arte Nova (USFAN) was reported. After carrying out a systematic review of the literature and interviews with the Presidents of the Clinical and Health Councils of five Health Centre Groupings, the measures identified were gathered and compared to the ones carried out by USFAN. Subsequently, the study identified the measures considered to be truly original executed by the health unit. Between the measures presented, the use of an access prioritization score was highlighted, as were daily briefing regarding the management of resources and equipment, qualification of the use of personal protective equipment, the project that aimed to optimize the use of paper;the use of a drive-through method to update the National Vaccination Program;and the daily training and sharing of information about COVID-19. In light of the current pandemic, innovative practices and tools have been created and carried out by the healthcare professionals in response to the growing needs of the population. This shows the resilience of these professionals and constitutes an opportunity to share and implement these tools in other healthcare facilities highlighting the continuing chance of improving. © 2023, The Author(s), under exclusive license to Springer Nature Switzerland AG.

ABSTRACT

The global COVID-19 pandemic made salient various paradoxical tensions, such as the trade-offs between individual freedom and collective safety, between short term and long-term consequences of adaptation to the new conditions, the power implications of sameness (COVID-19 was non-discriminatory in that all were affected in one way or another) and difference (yet not all were affected equally due to social differences), whereas most businesses became poorer under lockdown, others flourished;while significant numbers of workers were confined to home, some could not return home;some thrived while working from home as others were challenged by the erosion of barriers between their private and working lives. Rapid improvisational responding and learning at all levels of society presented itself as a naturally occurring research opportunity for improvisation scholars. This improvisation saw the arrival of a ‘New Normal', eventually defined as ‘learning to live with COVID-19'. The five articles in this special issue capture critical aspects of improvisation, paradoxes and power made salient by the COVID-19 pandemic in contexts ranging from higher-education, to leadership, to medical care and virtue ethics. In their own ways, each breaks new ground by contributing novel insights into improvisation scholarship. © The Author(s) 2022.

ABSTRACT

Aim: Since the early phases of the COVID-19 pandemic, health systems tried to adapt to ensure the continuity of care of oncological patients. This study aimed to describe the impact of SARS-CoV- 2 on rectal cancer screening and staging. Method(s): A two-year (March 2019 to March 2021) retrospective study concerning rectal cancer patients from a referral center was conducted. Patients clinical data from pre-COVID (March 2019 -February 2020) and COVID time (March 2020 -March 2021) was compared. Descriptive and inferential analysis was performed (Chi-Square test). Result(s): One hundred and sixty-five patients were discussed at the multidisciplinary meetings during the 2-year study period (mean age 69 years [+/- 11.1];M: 64%;F:36%). Upon comparative analysis both pre-COVID and COVID patients were found to have similar demographic characteristics, however during the pandemic a higher proportion of patients presented with low rectal cancers (36% vs. 42%;P = 0.1). Moreover, during the COVID period, fewer patients (minus 26%;npre-covid= 95 vs. ncovid = 70) were referred to the hospital, and a larger number of patients presented in Stage IV of the disease (17,9% (n = 17) in pre COVID period vs. 28,6% (n = 20) in COVID period (P = 0.07)). Lastly, the authors run a comparative sub-analysis between the above results and data from the 3 years prior to the pandemic (2017-2019) and still came across with lesser rectal cancer referrals during the pandemic year. Conclusion(s): Our data clearly shows that, during the COVID period, fewer patients received in-hospital care and a higher number were referred in Stage IV. This represents a red flag for the community and should alert the government to implement public health policies to reestablish colorectal cancer standard of care.

ABSTRACT

BACKGROUND: The COVID-19 pandemic triggered high mortality in the world population and resulted in impacts on health services. Services related to HIV care and prevention were discontinued and concerns about the impacts of COVID-19 on people living with HIV (PLHIV) were discussed in different countries. In this setting, the COVID-19 pandemic has provided a decrease in combination HIV prevention, which involves access and use of HIV services, including decreasing HIV pre-exposure prophylaxis (PrEP) prescriptions, post exposure prophylaxis (PeP), interruptions in HIV ongoing care of people living with HIV and access to HIV testing. The aim of this study is to assess the implications of the COVID-19 pandemic for combination HIV prevention. METHODS: A systematic review of observational studies was performed according to the Preferred Reporting Items for Systematic Reviews and MetaAnalyses (PRISMA). Searches were conducted in PubMed, Embase, CENTRAL, Lilacs and OpenGrey, which included studies on the impacts of the COVID-19 pandemic on prevention and care services for patients living with HIV, namely: PreP, PeP, antiretroviral treatment and testing, published until October 2021. The main impacts on these services were analyzed according to the average income of the countries. Data from the included studies were independently extracted by four reviewers. For the final articles that were selected, the following data was extracted: year of publication, study design, study site, sample size, mean age of participants, type of PrEP, adaptations to services during the pandemic and country income. RESULTS: The search identified 1396 studies, of which 90 were eligible. Most studies were cross-sectional (n=30, 32.97%) and qualitative (n=33, 36.26%), mostly performed in high-income countries. USA had the largest number of studies (n=35, 38.46%). The main themes of the selected studies were related to the repercussion of the pandemic on the health of people living with HIV (n=21, 23.08 %), in the combined prevention of HIV (n=18, 19.78%), testing and use of PrEP (n=11, 12.09% -each) and adaptations in services were also reported to maintain the provision of care (n=11, 12.09%). The most used strategy for offering the service was telehealth for consultations related to the treatment and monitoring of PrEP users. The results suggest regional discrepancies and according to the average income of the countries. Low-and middle-income countries were more affected and populations were more exposed to discontinuity in treatment and access to combination HIV prevention. It was observed that qualitative studies predominantly assessed changes in HIV prevention and treatment services. CONCLUSIONS: The Covid-19 pandemic had an impact on reducing the supply of combination HIV prevention. Systems of healthcare need to adopt adaptations in the health services in order to establish effective strategies to increase testing and access to telehealth for the population living with HIV, specially in low income countries.

ABSTRACT

Like other institutions, libraries were not prepared to respond to everything that the Covid-19 pandemic brought: social isolation, remote work, suspension of academic activities, uncertainties about the return to the new normal, etc. This paper analyzes information from six academic libraries in the Federal District (Brazil), describing what were the first measures and steps that libraries implemented to keep up with the current life of their users. Semi-structured interviews and document research with content analysis were applied, identifying the following results: (1) Adoption of protocols and guides for users on security measures, libraries published documents with security measures, based on information from international and national institutions;(2) Use of social networks to promote services and communicate with users, with Instagram being the main ally for the dissemination of services and for being in contact with users.

ABSTRACT

Coronaviruses have emerged as important agents of human infection. SARS-CoV-2, the causative agent of COVID-19, has triggered a global pandemic with devastating consequences. The understanding of fundamental aspects of these viruses is of extreme importance. Fast vaccine development has been a crucial factor in preventing serious disease, but the fast-paced emergence of new variants raises many problems. Viral non-structural proteins are fundamental for viral replication. SARS-CoV-2 nsp16 is a 20-O-methyltransferase with a pivotal role in Interferon antagonism. Nsp16 methylates viral RNA to mimic the host mRNA and then the cell stops recognizing it as foreign. This activity is stimulated by the interaction with nsp10. This protein also acts as a co-factor for the exoribonucleolytic activity of nsp14. Nsp14 also has significant anti-interferon importance that stems from its 2 distinct activities: the N-terminal 3'-to-5' exoribonuclease (ExoN) and a C-terminal N7-methyltransferase (N7-MTase). Unlike Spike proteins, these nsp10, nsp14, and nsp16 are highly conserved among viral variants. In this work, we are studying them and finding inhibitors in order to develop new therapies. Nsp10 is the prime target of our focus since it is the central player in the regulation of both nsp14 and nsp16.

ABSTRACT

In the management and organization studies literature, a key question to explore and explain is that of the family as an organizational stakeholder, particularly when working-from-home became the “new normal”. Departing from meta-analytic studies on the work-family relation and connecting with scholarly conversation on work-family boundary dynamics, we identify three main narratives. In the separation narrative, work and family belong to different realms, and including the family in the domain of organizational responsibility is seen as pointless. The interdependence narrative stresses that organizations and families are overlapping domains in which it is important to acknowledge that the policies and practices of the former might have an impact on family life, and vice-versa. The embeddedness narrative, brought to the fore by the COVID-19 pandemic, sees employment and family as progressively convergent and hybrid work domains. The evolution of employment relations towards increased hybridity of the work situation being embedded in the familial/household context increasingly calls for consideration of the family/household as an integral rather than a peripheral stakeholder. © 2022, The Author(s).

ABSTRACT

The editorial comments that covid-19 pandemic affected libraries and many of them, were caught by the back foot, that is, they had to close their doors and left their users helpless! Therefore, digitization and the creation of a digital library is not the future of libraries, it is the present. The library needs to digitize its old materials to save space, the ability to start a digital collections management project is becoming an essential skill for today's librarian. In addition, articles included in first issue of 2022 of the Ibero-American Journal of Information Science are commented.

ABSTRACT

This study aimed to explore how contextual variables associated with the period of confinement during pandemic can differentiate symptoms of anxiety, depression, and stress. It also intended to understand the concerns that were experienced by athletes when resuming their sport practice and how these could be associated with psychopathological indicators. The sample comprised 265 athletes who completed self-reported questionnaires. Results demonstrated that there were differences in psychopathological indicators in terms of hours of training per week, and the type of contact that athletes have with their coach. Athletes reported concerns regarding the resumption of sport practice, and that were associated with anxiety, depression, and stress symptoms. This study may contribute in warning the sports community about the impact of contextual variables-related to critical moments of this pandemic. Sports communities should encourage the preservation of the athlete's physical activities, as well as supporting coaches in continuing to guide athlete's training.

Introdução: Na tuberculose pulmonar (TBp), os principais padrões radiológicos são cavitação e nódulos centrolobulares com padrão de árvore em brotamento, predominantemente nos lobos superiores, e espessamento das paredes brônquicas. No entanto, podemos encontrar manifestações radiológicas atípicas como os tuberculomas pulmonares, tornando o diagnóstico, nestes casos, mais desafiador. Geralmente, o tuberculoma pulmonar aparece isolado e ocorre em cerca de 5% dos casos de tuberculose pulmonar. Nódulos múltiplos, que mimetizam neoplasia pulmonar ou metástases, por sua vez, são ainda mais incomuns. Descrição do caso: Trata-se de uma paciente do sexo feminino, 43 anos, hipertensa, asmática e com diagnóstico de espondiloartrite com início do quadro, do qual não soube precisar data, caracterizado por tosse seca diária e eventualmente com secreção branca ou hemoptise, que piora à noite e melhora parcialmente com anti-histamínico. Há cerca de 1 ano e meio, apresentou dor em dorso e precórdio, que piorava ao esforço físico e tosse, sem fatores de melhora. Há 10 meses, apresentou dispneia aos pequenos esforços, quando teve diagnóstico de COVID-19, porém os sintomas pulmonares se exacerbaram progressivamente. Relata também sudorese noturna e perda ponderal de 7 kg em 3 meses. Em março de 2021, realizou PET CT que evidenciou nódulos pulmonares múltiplos em lobos inferiores e superior direito, sem atividade metabólica específica e de etiologia a esclarecer. Foi realizada segmentectomia pulmonar à direita, no entanto a análise histopatológica da peça revelou processo inflamatório crônico e extensas áreas de necrose caseosa em segmento de lobo inferior direito, sugerindo diagnóstico de TBp, sendo encaminhada para nosso serviço especializado. Foi descartado o HIV. Após 4 meses de uso de rifampicina, isoniazida, pirazinamida e etambutol, 4 comprimidos por dia, paciente refere melhora da tosse seca e da sudorese noturna, além de melhora da adinamia e astenia. Comentários: Geralmente, nódulos pulmonares múltiplos são malignos, chegando a responder a 80% dos casos. No entanto, tuberculose pulmonar no Brasil e na região nordeste, deve sempre ser considerada como diagnóstico diferencial, mesmo diante de imagem não típica.

Introduction: Since the emergence of coronavirus disease 2019, known as COVID-19, there are limited data describing the risks and specific effects of severe acute respiratory syndrome coronavirus 2, or SARS-CoV-2, in pregnant women and their neonates. To date, there is scarce evidence of in utero transmission, and the risk of perinatal transmission and consequent neonate risk of developing COVID-19 during the perinatal period are also unknown. Methods: Since April 2020, universal screening of all pregnant women presenting in labor has been implemented in our hospital. To those who are positive for SARS-CoV-2, the possibility to room-in with the neonate is given. Neonates are tested for SARS-CoV-2 within the first 24 hours and again at 48 hours of life. We made a prospective cohort analysis of all neonates born of mothers positive for SARS-CoV-2 in our hospital from April to November 2020. Demographics, neonatal and maternal symptoms at the delivery, during hospitalization and once discharged, perinatal routine care and SARS-CoV-2 reverse transcription polymerase chain reaction results were studied. Results: We analyzed 23 mothers positive for SARS-CoV-2. Only eight declared having symptoms, mainly headaches, anosmia, and ageusia/dysgeusia. The 23 neonates were negative for SARS-CoV-2 in both reverse transcription polymerase chain reaction tests performed. All of the neonates were born and remained asymptomatic during the 14 days of surveillance time and all of them were breastfed. Discussion: According to our results, rooming-in was safe for newborns of SARS-CoV-2 positive mothers since there was no evidence of perinatal infection. This practice is a well-known way to promote early breastfeeding and encourage maternal-infant bonding. © Author(s) (or their employer(s)) and Portuguese Journal of Pediatrics 2021.

ABSTRACT

The so-called fourth industrial revolution brought a disruptive change in the way that communication technologies, distributed systems, intelligent data management, analytics and computational capability and other technologies are integrated to enable new functions and enhance capabilities not only to production systems, but also in many other domains such as education. Mobile Health (m-Health) education is one of these, where the number of applications and tools for m-Health education is extensive. The SARS-Cov2 (Covid-19) pandemic brought to life immense challenges towards education, technology, and the symbiosis with medicine. This paper introduces 31 of the current state-of-the-art m-Health education applications and analyses the results of an an inquiry to students and junior doctors during the confinement, designed to understanding their knowledge, use and trust regarding these apps. The results show that several applications are well perceived by their users and deserved their trust andconfirms a good relation between use and trust on the applications analysed. ABSTRACT

Introduction: Given the pandemic circumstances, fear may emerge due to uncertainty, the possibility of being infected by SARS-Cov 2 or infecting others. It may also be associated with psychopathological symptoms and impact quality of life resulting from the COVID-19 pandemic. Objective: The current study aimed to examine the factor structure and psychometric properties of the Fear of Contracting COVID-19 Scale (FCCS) in Portuguese adolescents and analyze the relationship between fear of contracting COVID-19 and depression, anxiety and stress symptoms, and with the quality of life of this age group. Method: The sample comprised 269 adolescents (137 boys and 132 girls), aged between 11 and 16 years old, attending the 3rd cycle of basic education. Participants completed a sociodemographic questionnaire, the FCCS, the Depression, Anxiety and Stress Scales (DASS-21), and the KIDSCREEN-10, as a measure of the quality of life in adolescents. Results: The FCCS revealed, as in its original version for adults, a one-dimensional structure, with good adjustment indicators and good reliability. The fear of contracting COVID-19 was associated with anxiety, depression, and stress symptoms, but it did not show a relationship with the adolescents' quality of life. Gender differences showed that the girls present higher levels of fear of contracting COVID-19, more anxiety, depression and stress symptoms, and a worse perception of their quality of life. Conclusions: The Fear of Contracting COVID-19 Scale showed to be suitable for use with adolescents. The fear of contracting COVID-19, although associated with psychopathological symptoms of anxiety, depression and stress, does not seem to be related to the quality of life of adolescents.

ABSTRACT

Introduction Les patients atteints de maladies musculosquelettiques inflammatoires/auto-immunes (I-RMD) n’ont pas été inclus dans les études de tolérance des vaccins contre le SARS-CoV-2 et sont souvent inquiets quant à la tolérance de la vaccination. Notre objectif est d’étudier la tolérance des vaccins contre le SARS-CoV-2 chez les patients atteints de maladies musculosquelettiques inflammatoires/auto-immunes (I-RMD). Patients et méthodes Pour cela, nous avons créé avec l’EULAR un registre international de cas rapportés par les médecins rhumatologues et internistes de patients atteints d’I-RMD et de RMD non inflammatoire (NI-RMD) vaccinés contre le SARS-CoV-2. Du 5 février 2021 au 27 juillet 2021, nous avons recueilli des données sur la démographie, la vaccination, le diagnostic de RMD, l’activité de la maladie, les traitements immunomodulateurs/immunosuppresseurs, les poussées, les événements indésirables (EI) et les infections COVID-19 chez les patients vaccinés. Les données ont été analysées de manière descriptive. Résultats L’étude a inclus 5121 participants de 30 pays, la majorité de France (40 %), Italie (16 %) et Portugal (14 %), 90 % avec des I-RMD (n=4604, 68 % de femmes, âge moyen 60,5 ans) et 10 % avec des NI-RMD (n=517), 77 % de femmes, âge moyen 71,4 ans. La polyarthrite rhumatoïde (33 %), les connectivites (18 %), les spondyloarthrites (11 %), le rhumatisme psoriaqique (10 %) et les vascularites (12 %) étaient les diagnostics les plus fréquents ;54 % des patients ont reçu des traitements de fond synthétiques conventionnels (csDMARD), 42 % des DMARD biologiques ou ciblés et 35 % des immunosuppresseurs. La plupart des patients ont reçu le vaccin Pfizer/BioNTech (70 %), 17 % AstraZeneca/Oxford et 8 % Moderna. Une infection COVID post-vaccination a été signalée dans 0,7 à 1,1 % des cas, selon le statut vaccinal (entièrement/partiellement vacciné) et le groupe RMD. Des poussées d’I-RMD ont été signalées dans 4,4 % des cas (0,6 % de poussées sévères), dont 1,5 % ont entraîné des changements de médicaments. Des EI ont été signalés dans 37 % des cas (37 % I-RMD, 40 % NI-RMD), des EI sévères dans 0,4 % des cas, très divers et avec une fréquence comparable et même inférieure à celle observée chez les patients atteints de NI-RMD (1,1 %). Discussion La tolérance au vaccin n’était pas différente entre les groupes I-RMD et NI-RMD. Dans les essais cliniques de vaccins à ARN contre le SRAS-CoV-2 dans la population générale, les taux d’EI graves étaient très semblables à ceux de notre étude, allant de 0,4 % à 0,6 % dans le groupe vacciné et de 0,5 % à 0,6 % dans le groupe témoin, ce qui suggère que ces EI graves ne sont pas nécessairement liés au vaccin. Conclusion Il s’agit de la plus grande étude de la tolérance des vaccins anti-SRAS-CoV-2 chez près de 5000 patients atteints de maladies inflammatoires/auto-immunes rhumatologiques. Le profil de sécurité des vaccins contre le SRAS-CoV-2 chez les patients atteints d’I-RMD était rassurant, et comparable à celui des patients atteints de NI-RMD. La majorité des patients ont bien toléré leur vaccination, avec de rares poussée d’I-RMD et de très rares EI sévères probablement non liés à la vaccination. Ces résultats devraient rassurer les rhumatologues et les personnes vaccinées, et favoriser la confiance dans la sécurité du vaccin COVID-19 chez les patients atteints de I-RMD.

ABSTRACT

The article is the result of research carried out in a music school, in undergraduate courses. The objective was to know which desire sustains teachers and students in the process of teaching and learning music. The concept of psychoanalysis desire was transversal to that of music education to deal with the theme. In the literature review few publications were found. Three professors and three students were interviewed, individually and collectively, through the Clinical Method of Psychoanalytic Orientation and Conversation, derived from this method. All interviews focused on the online format, due to the isolation forced by the Covid 19 pandemic. At first, we tried to locate in each interviewee the symptoms and significant present in the relationship with music in the family environment and then in the music schools. The following conclusions were reached: the traditional "family support" for the study of music did not necessarily mean support for desire, but its demand. The desire, as Lacanian psychoanalysis understands, was located more in presenting itself to the other as a musician, seeking the feeling of belonging, than in the interest in music as a language or aesthetic object. In this demand for music, as a way of rescuing a social bond, there as a real desire, the account that most appeared at the end of the interviews was the "fear of the stage", as a symptom of a dialectic between demand and desire for music.
